Whats the difference between an old item and a regular item? The stats don't seem to be different?

You can not compound old accessories to make +1s.

but you can compound 3 OLD unique ( same one) and make a unique normal +0
it works only with OLD uniques that don´t seal in your account when you use it.
Exemple , old ring courage , elf belt , and others if you get 3 of them and try compound , it will show ' wrong material '

and where do you get the items required to compound them?

got scared for a minute, seeing a thread with thing in it.

anyways, you need 5 nest scraps and 50 scraps of steel per try. Both come from monster stone monsters. Also you need a soft stone, which the NPC who does the compounding will sell to you for 1M I believe?

The success rate to compound old uniques are extremely low though from what I've seen
