Most of you might know &fmt=18 (HD quality); however, new version of format has been released which is &fmt=22
http://www.youtube.com/watch?v=g8E2IFbwO3w (without 18 or 22. shitty quality)
http://www.youtube.com/watch?v=g8E2IFbwO3w&fmt=18 (much better but still blurry)
http://www.youtube.com/watch?v=g8E2IFbwO3w&fmt=22 (blurry is gone, video is pretty much perfect)
Give it a shot.
*Reminder* It doesn't work for all videos on YouTube also if you recorded your video in shitty quality don't expect YouTube to make it perfect. What you record is what you get. Start using H264 format for your future recordings.
